My Aged Care
My Aged Care is the entry point to access aged care services including information about:
- The different types of aged care services available
- Eligibility for services
- Referrals to service providers that can meet your needs
- Your contribution to the cost of your aged care.
The Australian Government contributes to the cost of aged care services. However, individuals are expected to contribute to the cost if you can afford to depending on; your financial situation, the number and types of services you receive, and the service provider.
You can access My Aged Care via their direct telephone number My Aged Care 1800 200 422 or you can visit the Community Care Centre, 22 Miller Street Gilgandra. The friendly staff will help you through your referral for an assessment to access Gilgandra Lifestyles Community Care services.
An assessor will ring you or your support person to arrange a time to visit you at home to talk with you about your needs. Once your assessment has been approved you can chose the service provider who will meet your particular circumstances.
All services can be accessed by those who are the most disadvantaged people in the community. They will need to be assessed for eligibility by telephoning Community Care on 02 6817 8750 to make an appointment or visiting the Community Care Centre.
